I agree that the Uniform loaned to my child must be properly cared for and returned as required. In case of loss or damage to the Uniform while loaned to my child, I further agree to pay for the repair and/or replacement of the Uniform. Should my child be found intentionally damaging the Uniform or not maintaining the Uniform properly, I agree that the Uniform will be returned on demand to the school. In this event, the student will be excluded from participation with the team. I understand that my child is not to wear the Uniform at any time other than that prescribed by the coach. I further understand that the Uniform is to be worm only in the manner prescribed by the coach.
