Athletic Program

Athlete Eligibility Check Sheet

All of the following requirements must be met before any student will be allowed to practice or participate in ISPS Athletics. (Student should check off requirements as met)

1)    ________ Physical Examination Form: Must be completed, signed by both parents and the physician and either submitted to the coach or on file in the nurse’s office. A physical must be given for each school year in order to participate in sports for that given year.

2)    ________ Parent Permission Form: Must be completed and signed by the parents and submitted to the coach to be eligible.

3)    ________ Parent/Student Compliance Form: Must be signed by both parents and student and returned to the coach to be eligible.
